Have you looked into wether she is being bullied at school, she could also have seperation issues and this could also be caused by stress, it is a big event starting school and as she is only doing it there and not when she is at home which to me says that this posibly the case.
Talk to the school and see if they can help pin point any issues , please dont shave her hair as this will cause her more stress, also see if you could talk to the school nurse and your doctor again and see if they could refer you to a child psychologist or similar

It sounds like your daughter might have Trichotillomannia, or compulsively pulls her hair.
